Bermuda medical records: use the BHB HIMS route
For medical records held by Bermuda Hospitals Board, use Health Information Management Services and the relevant KEMH or MWI authorisation form. The separate PIPA Subject Access Request form explicitly says it is not the medical-record request route.
Prepare the official request
- Identify which records and dates you need and whether KEMH or MWI holds them.
- Open the corresponding authorisation form from the official HIMS page.
- Complete the request and submit it with proof of identity through the official HIMS channel.
- Follow the fee assessment and retain the receipt.
- Wait for notification, then bring the required identification and receipt to collect the records.
Keep the right evidence
BHB asks for at least 48 hours’ notice; that is not a promise that every request will be complete within 48 hours. Contact HIMS if you are unsure which records to request or what authority is needed for another person.
A birth certificate is not part of the medical record even if the birth occurred at KEMH. The Registry General supplies that civil record. A PIPA request for other personal information and a HIMS medical-record request also serve different purposes, so choose the right form before gathering identity attachments.
If a recipient accepts a personal working copy, you can hide agreed unnecessary fields in a separate reference copy. Check repeated details, codes and every page. The exported image-based file does not preserve selectable PDF text or an original digital signature. Keep this output separate from any original, certified copy or unchanged official file required for submission.
For the civil record, see Bermuda birth certificates. For a different identity-check dossier, see Bermuda police clearance.
Questions
Can I use the PIPA Subject Access Request form for medical records?
That form directs medical-record requests to HIMS using the KEMH or MWI authorisation form.
Will HIMS supply my birth certificate?
BHB says birth certificates are not held in the medical record. Contact the Registry General for that certificate.
What should I bring for collection?
The HIMS instructions require the applicable release evidence, proof of identity and payment receipt. Follow the notification for your request.
