Identify the BHB personal-information requester
Bermuda Hospitals Board’s Subject Access Request Form names a passport or driver’s licence as proof of identity. Ask the Privacy Officer which sides and format are needed for your request.
Prepare the requested PDF
- Choose the requested passport page or driver’s-licence image. Include a reverse only if needed for that same document.
- Select Passport or Bermuda driver’s licence. Adjust the crop to retain the document’s complete edges.
- Choose PDF and check the page order. Letter is the starting sheet size; switch to A4 if requested.
- Download and reopen the PDF. Confirm your name, photograph and required identifying details are clear.
Personal-information and medical-record requests differ
The BHB form directs medical-record requests to HIMS or the MWI release process. Prepare the identity attachment for the form you are actually using, rather than treating this tool as a records-request service.
The licence preset uses a full-page layout. The passport preset uses a standard 125 by 88 mm page; a document photo is not an official electronic passport.
This tool handles one document and its reverse. Keep a separate authority letter or another person’s evidence in the format the Privacy Officer requests. It does not merge an entire request bundle.
Check the saved copy
Choose JPG, PNG, supported HEIC or PDF up to 25 MB. Only the first ten pages of an input PDF are processed. Download JPG or an image-based PDF; selectable text, fillable fields and digital signatures are not preserved. Keep any original signed or fillable file.
The document is processed in your browser without an account or document upload. The downloaded copy removes camera metadata and leaves the source unchanged.
Use Photo to PDF for the layout, Compress for an attachment ceiling, Watermark for an agreed purpose label or Hide details for agreed unnecessary fields. Save between tools and inspect the final downloaded file.
Common questions
Can I make one PDF of the required licence sides?
Yes. Add the reverse to the same document, select PDF and review both pages. Ask whether both sides are actually needed.
Does conversion submit the BHB request?
No. Send the completed official request and required identity evidence through BHB’s stated channel yourself.
Will the PDF keep digital signatures or fillable fields?
No. Every output PDF page is an image. Selectable text, interactive fields and digital signatures are not preserved; keep the original file when those features matter.
